What Is Exposure & Response Prevention (ERP) Therapy?
Exposure & Response Prevention, commonly called ERP, is a structured form of therapy specifically designed to treat OCD and related anxiety concerns. ERP is based on the idea that avoiding fears or performing compulsions can keep the OCD cycle going.
For example, someone with contamination-related OCD may feel intense anxiety about touching a doorknob and immediately wash their hands to feel safe. The handwashing may temporarily reduce anxiety, but the brain can begin to learn that the compulsion is necessary to prevent something bad from happening. Over time, this can make the fear and compulsion stronger.
ERP takes a different approach. With the support of a therapist, you gradually practice facing uncomfortable thoughts, feelings, or situations while choosing not to engage in the compulsive response. Over time, this process can help you learn that anxiety and uncertainty are uncomfortable but manageable and that you do not have to let OCD control your choices.
How Does ERP Therapy Work?
ERP therapy is not about forcing you into situations before you are ready. Instead, treatment is collaborative and personalized to your specific concerns, symptoms, and goals.
During therapy, we work together to identify the thoughts, situations, or experiences that trigger your OCD symptoms. We may then create a hierarchy, which is a list of situations that cause different levels of anxiety. The “exposure” portion of ERP involves intentionally facing a feared thought, situation, object, or sensation. The “response prevention” portion involves resisting the urge to perform the compulsion or ritual that you would typically use to reduce anxiety.
This process may feel uncomfortable at first, but the goal is not to eliminate anxiety immediately. Instead, ERP helps you learn that you can experience anxiety without needing to respond to it with compulsions or avoidance. With practice, many people find that their anxiety becomes more manageable and that OCD has less influence over their daily lives.
